I don't know if we've ever come up with a firm policy on ad-hoc movie bands of the Blues Brothers/Backbeat variety, but here's another marginal case. The credits for the movie "Superbad" include:
Bootsy Collins & The Superbad Band
Bootsy Collins - bass, vocals
Bernie Worrell - clavinet, organ
Clyde Stubblefield - drums
John "Jab'o" Sparks - drums
Phelps "Catfish" Collins - guitar
Luis Conte - percussion
Jeff Babko - organ, clavinet, moog
Lyle Workman - guitar
Jerry Hey - trumpet, orchestrator

The band plays instrumentals, which are also included on the official soundtrack. The Myspace page for the soundtrack says the songs are "composed by Lyle Workman featuring Bootsy Collins and the Superbad Band":

Is that enough to warrant inclusion based on the soundtrack album (or maybe the movie DVD)?

I'd say so - as long as it was credited & 'released' on some sort of format you can keep (DVD, soundtrack, VHS etc). I'd not favour including a band that appear in the credits of a film but where the film was never available to buy.

At best I think we'd have to use the movie instead of the soundtrack album, given that the soundtrack seems to credit all of the songs to Lyle Workman:

Yeah, I just watched it, and the credits above are how they appear in the scroll at the end of the movie.
As for the CDUniverse credit for Workman, I believe that's the composer's credit. The Myspace page for the soundtrack certainly suggests that the band is credited as such, but I guess someone would have to track down the album to make sure.
