What should we do with this one?

It's a bonus disc that was available only with a limited edition of a Greatest Hits from the Smashing Pumpkins. Should we have an entry for the original album and one for the limited-edition? Or an entry for the original album and one for an album that was never available seperately, called 'Judas Ø' (but, AFAIK doesn't have a cover).

Since the bonus disc is not available outside of Greatest Hits, I'd say that there are simply two versions of Greatest Hits. Given that, then I'd think there's no need to include an additional entry for it unless there are alternate lineups on it that are not on the standard version.

I'd go with a single entry for the Greatest Hits as if all of them had the bonus disc. I can think of a number of circumstances where material could be removed from an album in later pressings but we wouldn't distinguish those releases.
