This looks to be the only time k.d. lang appears without her dots on the front cover of a non-promo release:

However, the rendering on the front cover is kind of stylised, and she still has her dots on the disc and spine. Should she still lose them from the primary name? It'd seem overly harsh in this case.

The rules are pretty unforgiving, and there's no question about the lack of dots on the cover. Even if her default solo artist name loses the dots because of that, the search mechanism will come through on searches for k.d. or kd. I'd just go with what we see on the cover.
